We are currently recruiting for full-time a Senior Medical Social Worker for our Beaumont Lodge Transitional Care Unit, Coolock, Dublin 5.
Purpose Of the Post
The Senior Medical Social Worker post in the Beaumont Lodge Transitional Care Unit is part of a new service development at Bartra Healthcare, which aims to provide service to patients transferred from acute hospitals and who require ongoing MDT input prior to discharge. The purpose of this post is to provide a comprehensive Social Work service to patients and their families in Beaumont Lodge. The key focus of this post will be promotion of independence to patients based on a multidisciplinary directed SMART bespoke goal setting model
As a member of a multidisciplinary team, senior medical social worker will be required to:
- Approach their work with the flexibility and enthusiasm necessary to make this principle a reality for every patient to the greatest possible degree.
- To provide a comprehensive social work and counselling service to all patients and their families of the Beaumont Lodge.
- To develop strong co-working multi-disciplinary professional relationships that foster person centre patient care collaboratively.
- To develop robust and comprehensive patient Social Work care plans in partnership with the patient, their family and the multi-disciplinary team in VCARE to effect detailed and safe transfer to the community setting.
- Engage in peer supervision and lead peer supervision sessions.
- To provide counselling, emotional and practical support to patients and their families which will assist them in developing strategies to cope with the psychosocial and emotional impact of illness
- Engage in a buddy system of best practice with colleagues and lone working that is safe and supported
- To be a resource/knowledge base to the MDT regarding the psychosocial, systemic/environmental aspects of the patient’s life which may impact on ability to cope with illness.
- Assist/develop/collaborate in discharge planning development for the patient in partnership with the patient and their family in the context of multi-disciplinary meetings.
- To consult and liaise with other agencies including community agencies
- Ensure the delivery of social work services in accordance with legislation, policies
- and procedures, guidelines and protocol.
- Participate in development of Medical Social Work in Beaumont Lodge and in keeping with the core principle of knowledge and learning to enhance service delivery.
- Keep a high standard and accurate records in line with best practice, GDPR and FOI, CORU Code of Ethics and Standards and HSE policies, protocols and guidelines and best practice standards.
- Perform other duties based on service need as may be assigned from time to time by the Principal Social Worker and or other nominated staff member.
Qualifications, experience & knowledge:
Required:
· Must be registered in the Social Work Register maintained by the Social Work Registration Board at CORU.
· Maintain live annual registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU.
· Must have the requisite knowledge and ability for the proper discharge of the duties of the office.
Desirable:
· Evidence of continuous professional development relevant to the required area of specialisation.
· Have minimum 3 years of relevant post qualification experience.
